What is fragmentation on a hard disk?
Think of your hard disk as a bookcase with books. Lets say you love book series and books with many parts , when you bought them you organized them in order . Everything was perfect at the start , all nice and clean , then you started removing books, maybe lending your friend a book (that is a part of series) , maybe you forgot to put a book back and maybe you lost some books . The bookcase starts having some empty spots and because you might not care much you started filling those empty spots with new books or a book your friend gave you back or you found that one book you lost some weeks ago. You might not really care about organizing them . After a bit of time the bookcase is a mess and trying to find the book you searching is get harder and harder to find . What do you do to solve that problem? You start moving around books so , for example , all the Harry potter books are together in series. You may now ask me "HEY WHAT DOES THAT HAVE TO DO WITH MY HARD DISK AND FRAGMENTATION AND FRAGMENTATION". It is simple , the hard disk is the bookcase , the programs are the book series , the books are the data (of those programs) and and you searching the books is a system in the hard disk that searches data. So in simply put , fragmentation is when your hard disk is a mess and defragmentation is when your hard disk is cleaning up and organizing itself.
A small note : fragmentation and defragmentation does not have any connection with the way folders are organized but it is the result of installing and uninstalling or removing and adding stuff on your hard drive.

the guide to deFRAGMENTATION in windows
1)On the bottom-left click on File Explorer
2)Press the right click on your hard disk (local disk by default , or the C: letter drive)
3)A drop down menu will appear , click the last option called "Properties"
4)A window will appear , click on the "Tools" tab
5)On the "Tools" tab press optimize as the image 6 shows
6)A new window will appear , choose the hard disk you want to defrag and press optimize (or analyze if you want to see in what condition your hard disk is)
7)Wait until it shows OK in "Current status" like in image 12
If you want a better defrag program that is more advanced (it also has visual representation) i suggest Auslogics DiskDefrag
